The short answer

People with narcissistic personality disorder rarely spend hours worrying that they might have it. Repeatedly checking whether you are selfish, manipulative or a bad partner is far more often a sign of anxiety, and frequently of OCD. The theme is sometimes called moral OCD, or a form of scrupulosity.

What it looks like

  • Replaying conversations to check whether you were cruel or self-centered
  • Taking online narcissism quizzes, then retaking them
  • Asking a partner or friend for reassurance that you are a good person
  • Avoiding speaking up, saying no or asking for things, in case it proves you are selfish
  • Over-apologizing and people-pleasing to compensate
  • Feeling the relief of reassurance fade within hours

Why reassurance doesn't work

Each time you check, quiz or ask, the anxiety drops for a while. That relief teaches your brain the question was urgent, so it comes back stronger. No answer is ever certain enough for OCD, which is why the hundredth reassurance feels the same as the first.

How therapy helps

Exposure and response prevention (ERP) is the main treatment. Instead of trying to prove you are a good person, you practice living with the uncertainty and resisting the checking and reassurance. Over time the question loses its grip. If the fear grew out of an unpredictable or critical childhood, trauma work such as EMDR or parts work can help with the beliefs underneath.

Questions

Can someone with OCD actually be a narcissist?

Diagnoses can co-occur, but persistent fear and guilt about being a narcissist point away from it. A proper assessment can give you clarity.

Should I take a narcissism test online?

If you have taken several already, it is probably working as a compulsion. Notice the urge and try not to act on it.

Is this the same as having a narcissistic parent?

Not necessarily. Some people with this fear grew up around narcissistic behavior; many did not.
